We had a fun time celebrating Halloween with everyone in our nursing home. Thank you to all who dressed up and visited us during Halloween week. We are so thankful for the involvement of our Avon community!

Avon Place Skilled Nursing and Rehabilitation and its parent company, Foundations Health Solutions, follow the vision of nursing home developer Brian Colleran to create a "Culture of Care" in our nursing homes and communities. Whether in Avon or our communities across Ohio, we strive for excellence in long-term care, short-term care, hospice care, aquatic therapy, and a variety of other services.
